The 'End of Life Care Hub' was launched in Creswick on Wednesday 8th August 2018.
The End of Life Care Hub (EoLC Hub) is a core element in supporting a Compassionate Community where everyone knows how to care for someone who is caring, dying or grieving and knows where to access supports.
With this philosophy in mind, the hope is that the EoLC Hub will be a model for other regional communities to embrace and duplicate.
The Hub has been designed to be an open and welcoming place to come and ask questions and be connected with existing services that care for people and families with terminal or life limiting illnesses. We have a free resource library to browse on end of life care and palliative care issues The Hub also offers assistance in honouring a person in death and to respect and support their family and friends in bereavement.
The EoLC Hub works in collaboration with a number of services and practitioners in the community including: Funeral Planners, Reiki Masters, Masseuses, Meditation Classes and Counsellors. We are working with Art, Music and Pet Therapy to make this more accessible in our community and have people trained to help you complete you Advance Care Plan.
The therapies and services on offer at Creswick Hub change as the communities needs change.
